Steps
Follow these requirements carefully before submitting your application.
1. Locate Docs
Locate all relevant documents to present at the Consulate. Examine all your documents carefully before submitting your application.
2. Verify
Photocopies or copies certified by a Notary Public will not be accepted.
Photographs must be taken at a PROFESSIONAL photo studio. Instant photos are not acceptable.
3. Submit
Applicants MUST submit a legibly completed application form in full. On PAGE TWO Applicants must also sign the declaration in the signature box. BOTH signatures MUST be identical.
- All documents that are submitted for passport applications should be originals or certified copies.
- Original documents in Spanish must be accompanied by a certified English translation.
- Laminated documents will not be accepted.
The average processing time for passport applications that are submitted through the Embassy is 10 weeks after PICA receives all the relevant documents. Non-compliant passport applications will be delayed, therefore, applicants are encouraged to carefully review the list of required documents prior to submitting an application.

Important Guidelines for In-Person Appointments
- Only the applicant is allowed to be inside the interview room at the time of the scheduled appointment. All other visitors accompanying the applicant should remain within the waiting area. In the case of minor applicants, only the parent/legal guardian giving consent will be permitted to accompany the child to the interview room.
- The use of cellphones in the interview room is strictly prohibited.
- Please ensure that the relevant application form is printed and completed and all other supporting documents are on hand prior to the scheduled appointment time, to ensure the smooth and timely flow of appointments for the day. If your appointment time has begun and the application form is deemed incomplete by the Consular Officer conducting your appointment, you may be asked to return to the waiting area to complete the form and the next scheduled applicant will be called in to begin their interview, leading to delays in the processing of your application.
- If you have a scheduled appointment and are no longer able to come in during the selected time slot, please reschedule or cancel your appointment through the booking site at least 24 hours before the scheduled appointment.

How to Apply
The Passport, Immigration & Citizenship Agency (PICA) regulations and guidelines govern all applications. The Consulate facilitates the process, but PICA is the final authority.
Applicants MUST present:
- A legibly completed application form signed in all required areas.
- All required documentation and valid identification.
Processing Period
- Mailed-in: 14 weeks from approval.
- Walk-in: 10 weeks.
Payments
Mailed-in: Money order or cashier’s cheque payable to the Consulate General of Jamaica – Miami.
Walk-in:

Cancellations
Must be in writing and signed by the applicant. For minors, the parent/guardian who submitted the application must sign.
Refunds
Refunds are not automatic and depend on the processing stage. Mailing & Consular fees will be deducted.
Your passport will be returned via USPS Express Mail. No P.O. Box addresses accepted.
Requirements
All supporting documents must be original. No photocopies.

Instructions
- Include your email address on Page 1 Section A.
- Sign inside the signature box on Page 2 (centered, not touching lines).
- Married persons must complete Section B.
- Section K is completed by the Consulate.
- For minors under 18: complete Section C; parent/guardian must sign.
- Parent/guardian must present valid photo ID.
- Tick the correct box in Section E and sign.
- Provide emergency contacts in Section F.
- Section G must be signed by Notary Public or Jamaican Official.
- Headgear for religious purposes requires completing Section H.
Photographs
Must be taken at a professional photo studio. No instant photos.
Signature
Sign in the center of the signature box. Signature must NOT touch the lines. Both signatures must match.
Birth Certificates
“Registry of Birth” document is NOT acceptable.
Certified birth certificates can be obtained from RGD – www.rgd.gov.jm.
Processing Time

Vital Records: Important Notes
Examine all your documents carefully before submitting your application. Ensure they are original. Photocopies or notarized copies will NOT be accepted. Documents will be returned after the process along with the passport/visa.
Proper Authority
All supporting documents (birth, death, marriage, divorce, name change, adoption, etc.) must be certified by the official records office in the district where the event occurred and must bear the original seal/stamp and signature.
Female Married Applicants
Marriage certificates must be obtained from:
- Registrar General’s Department (if married in Jamaica)
- Vital Records office or county court/registry in the US
- Appropriate Vital Records office outside the US
Foreign Language Documents
Must have official translation with original stamp/seal/signature.
Court Documents
Divorce, adoption, or name change documents must be certified by the issuing court with signature, title, record number, and date of certification.
Minor Passport (Applicants under 18 years)
First-Time Applicant
- Completed passport application form (Notarized at Section G)
- Completed “First Time Application Form”
- Two identical color photographs (one certified by Notary Public)
- Original Birth Certificate or Certificate of Citizenship or Adoption Register
- Parent’s valid ID (passport, driver’s license, voter ID; notarized copy if mailed)
- Parent/guardian must complete Section C and sign Section E
- Parent’s signature must match ID signature
- Legal guardian must submit court order or notarized consent letter with notarized ID copy
- Name change document if mother’s name differs
- Children 12+ sign in the box without touching borders
- All first-time applicants must apply in person

Minor Passport Renewal

Adult Passport
First-Time Applicants

- Original Birth Certificate / Adoption Register + letter on arrival in US (if born in Jamaica) or Citizenship Certificate (if born overseas)

Processing Time
Approximately 10 weeks in person; 14 weeks by mail. Notarization not required for in-person submissions.
Emergency Travel
Professional photography service is available at 44 W. Flagler if needed before arrival.
If you do not have a valid passport, you may apply for an Emergency Certificate. It is issued primarily for travel to Jamaica in cases of unforeseen immediate travel needs (illness, death, loss of passport while traveling, or unexpected business travel). Proof of the emergency is recommended.

FIRST-TIME PASSPORT APPLICANTS

- Completed passport application form. See Application Form
- Birth Certificate/ Adoption Certificate or Jamaican Citizenship Certificate.
- Two (2) identical passport-sized photographs that adhere to PICA’s photo guidelines. Please see PICA’s photo guidelines NB. One of the photographs should be certified by the same certifying officer that certified the passport application form.
- Valid Government issued photo ID (e.g. USA passport/Green Card, USA/Jamaican driver’s license, Jamaican Electoral ID) - If no ID is available, please see information regarding the completion of a Declaration of Identity (DOI) form.
- If the applicant was born in Jamaica, a signed, detailed statement (by applicant) outlining the circumstances surrounding the applicant’s entry into the USA (including document used and name used to enter the USA, landing date (to the best of memory), and any other pertinent details).
- Proof of name change if name has been changed (e.g. Marriage Certificate, Deed Poll, Divorce Decree, Court Order).
- Copy of proof of payment of application fee in the amount of US$95.00.
- Consular and postage fee in the amount of US$20.00.
The acceptable forms of payment are cash, or debit card (a surcharge is applied by the banking machine used for debit card) for in-person appointments, ONLY. For mailed in applications, payment must be made by way of Money Order made out to the Embassy of Jamaica. PICA is only able to issue passports to Jamaicans based on the accuracy of the information provided by applicants on the passport application form. If false or incomplete information is provided, your application will be delayed or be referred for further review and investigation.

Section G: Should be completed and certified by any one of the following certifying officials: Consular Officer at the Embassy, Justice of the Peace, Notary Public, Attorney-at-law, Medical Doctor or a Minister of Religion authorized to perform marriages.
Section H: Should be completed and certified by any one of the following certifying officials: Consular Officer at the Embassy, Justice of the Peace, Notary Public, Attorney-at-law, Medical Doctor or a Minister of Religion authorized to perform marriages.
If the certifying officer is not familiar with the applicant (known him/her for more than 12 months), he/she can indicate that the applicant was seen in person at the appropriate blank space at this section.

RENEWAL PASSPORT APPLICATIONS
All adult renewal passport applications that do not require a change in the name of the applicant should be completed using PICA’s online passport application portal
Passports with less than 12 months validity can be renewed. If the passport has more than 12 months validity, the applicant will be required to pay the fee for the replacement of the passport.
For adult renewal applications that require a change in the name of the applicant as well as the renewal of a minor passport to an adult passport, the following documents are required for the application process:
- Completed passport application form (Jamaica Passport Application Form)
- Birth Certificate/ Adoption Certificate or Jamaican Citizenship Certificate.
- Two (2) identical passport-sized photographs that adhere to PICA’s photo guidelines. Please see PICA’s photo guidelines NB. One of the photographs should be certified by the same certifying officer that certified the passport application form.
- Most recent Jamaican passport.
- Proof of name change if name has been changed (e.g. Marriage Certificate, Deed Poll, Divorce Decree, Court Order).

Parents are to appear in person at the Embassy of Jamaica in Washington or at the Jamaican Consulate in their country of residence with children 3 years and older. If mother is married, the Marriage Certificate is required.
Legal Guardians are required to submit relevant documentation showing legal guardianship along with a valid ID or a notarised letter of consent from the parent(s).

REPLACEMENT PASSPORT APPLICATIONS
Applicants whose most recent Jamaican passport was lost, stolen or damaged are required to apply for a replacement passport. The requirements for this type of application are outlined below.
- Completed passport application form (instructions on how to complete the passport application form provided below).
- Birth Certificate/ Adoption Certificate or Jamaican Citizenship Certificate.
- Proof of name change if name has been changed (e.g. Marriage Certificate, Deed Poll, Divorce Decree, Court Order).
- Two (2) identical passport-sized photographs that adhere to PICA’s photo guidelines. Please see PICA’s photo guidelines NB. One of the photographs should be certified by the same certifying officer that certified the passport application form.
- Valid Government issued ID.
- Completed Declaration of Identity if applicant has no valid government issued ID.
- Police report for lost or stolen passport.
- Copy of most recent Jamaican passport (if available).

- Consular and postage fee in the amount of US$20.00.

Parents are to appear in person at the Embassy of Jamaica in Washington or at the Jamaican Consulate in their country of residence with children 3 years and older. If mother is married, the Marriage Certificate is required.
Legal Guardians are required to submit relevant documentation showing legal guardianship along with a valid ID or a notarised letter of consent from the parent(s).
The acceptable forms of payment are cash, or debit card (a surcharge is applied by the banking machine used for debit card) for in-person appointments, ONLY. For mailed in applications, payment must be made by way of Money Order made out to the Embassy of Jamaica. Mailed-in applications are to be sent to: The Embassy of Jamaica, 1520 New Hampshire Ave, NW, Washington DC. 20036.
- To ensure that the passport application of your child is not delayed because of errors on the passport application form, please follow the instructions below.
- PICA is only able to issue passports to Jamaicans based on the accuracy of the information provided by applicants on the passport application form. If false or incomplete information is provided, the application will be delayed or be referred for further review and investigation.

Page 2The minor should sign inside the rectangular signature box at the top of the page, without the signature touching the lines of the box. (If the minor is under the age of 12 years, a signature is not required).
Section C: must be fully completed and date affixed by the Parent or Legal Guardian of the minor in question. The same Parent/Legal guardian is required to sign Sections C and E ensuring both signature samples are closely identical to each other. N.B. Also, the Parent/Legal Guardian’s signature at Sections C and E must be closely identical to the signature reflected on the identification being presented.
Section D: Should be left blank.
Section E: Tick “√” the second box bearing the inscription “All previous passports granted to me have been surrendered, other than Passport or Travel Document No… (insert passport number of most recent passport) …”. The Parent/Legal Guardian is required to sign Section E. When signing at Section E, the signature must be closely identical to the signature at Section C and both signatures at Section C and E must be closely identical to the signature on the identification being presented. Be sure to indicate the date on which you completed the application form.
Page 3Section F: Two emergency contacts must be supplied, one of which must be in your country of residence.
Section G: Should be completed and certified by any one of the following certifying officials: Consular Officer at the Embassy, Justice of the Peace, Notary Public, Attorney-at-law, Medical Doctor or a Minister of Religion authorized to perform marriages. The certifying official should complete the section in respect of the Parent/ Legal Guardian that is giving consent to the minor and not the minor. If the certifying officer is not familiar with the Parent/ Legal Guardian (known him/her for more than 12 months), he/she can indicate that the Parent/ Legal Guardian was seen in person at the appropriate blank space at this section.
Page 4Section H: Must be completed if the minor is required to wear head covering for religious reasons. Section I: Must be completed by all applicants born outside of Jamaica.
PAYMENT FOR PASSPORTS
All passport applications that are submitted to the Embassy for processing must be accompanied by a copy of the proof of payment that was made to the Passport, Immigration and Citizenship Agency (PICA).
- Passport fees can be paid to PICA via any of the following methods:
- Obtain a bank draft in the correct amount made payable to “Passport, Immigration and Citizenship Agency, 25C Constant Spring Road, Kingston 10, Jamaica”
- Have a relative/friend in Jamaica make the payment for you in US dollars at PICA’s offices in Kingston.
- Effect an international wire transfer payment through your bank using the following details.
- NB. Applicants should take into consideration the bank charges for the international wire transfer and send an excess of approximately USD37.00 to cover bank charges.
- Consular and postage fees in the amount of USD20.00 per passport application should be paid to the Embassy of Jamaica in US dollars cash or via deposit to the Embassy’s USD Bank account. A copy of the bank receipt should also be submitted with the passport application.

Jamaican nationals who are renewing their adult Jamaican passport without any changes to their name are encouraged to complete the passport application online via PICA’s online passport application portal.
You may complete the series of eligibility questions on the portal to determine whether you can renew the passport via this method. For the online passport renewal service, payment is done online directly to PICA.
EMERGENCY TRAVEL DOCUMENT
An Emergency Certificate (EC) is a temporary travel document that is issued to Jamaican nationals for emergency one way travel to Jamaica in cases where these nationals are without a valid Jamaican passport. In exceptional circumstances, an EC may be issued for travel to other countries.
The EC is subject to acceptance by the respective airlines and country of destination. It is recommended that you contact your airline before applying for this document.
The Embassy of Jamaica reserves the right to request additional documents in further support of an application. The issuance of an EC is not guaranteed.
